Have the right people in the right room with the right props at the right time, and let everyone not on the sheet know they have the day.

A theatre show report template — act timings, technical notes, company and performance notes, absences and covers, front of house notes and director's space.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.

Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Give the director, the producer and tomorrow's crew one honest page about what happened tonight, written while it is still clear, instead of a chain of messages the next morning.
Stage managers and deputy stage managers of plays, musicals, opera and dance, from a professional house to a school or amateur production.
One page, top to bottom:
Printed under the title: The stage manager's account of one performance — timings, what went wrong technically, what the company did differently, who was off, and what the director or producer needs to hear before tomorrow.
Write-on lines: Production · Venue · Performance date · Perf no. · Stage manager · DSM / on the book · House count · Report copied to · House open · Act 1 up · Act 1 down · Interval · Act 2 up · Act 2 down · Running time
Table, 4 rows: Act / cue · Dept · What happened · Action / by whom
Table, 4 rows: Who / scene · Note · Reported to
Write-on lines: Off tonight · Covered by · First aid used? · Understudy call needed? · Front of house — latecomers, walk-outs, phones, incidents
Because there is no such thing outside your own production. Write tonight's timings in and compare them with the previous reports; a show that is drifting longer is visible from the file, not from a printed target.
That is the company's decision, which is why there is a 'copied to' box rather than a list. Commonly the director, the producer and the heads of department, but the sheet does not assume.
Have the right people in the right room with the right props at the right time, and let everyone not on the sheet know they have the day.
Answer every question the band and crew will ask today before they ask it, on one page that can be photographed and passed round the bus.
Capture an incident properly on the day, in the affected person's own words, with the witness details and equipment notes that become impossible to gather later.
Get the day out of one person's head and onto a page everybody can read, so the event does not stall when the organiser is dealing with something else.
The same subject, with a version you can fill in on screen.
