What this sheet is for

Capture an incident properly on the day, in the affected person's own words, with the witness details and equipment notes that become impossible to gather later.

Who it is for

Workplaces, clubs, halls, sites, schools and any premises where something can go wrong and someone will later ask what happened.

How to use it

  1. Fill it in the same day. Detail you are certain of now is detail nobody has next month.
  2. Record what was said and seen rather than what you concluded.
  3. Take witness contact details before people leave, not afterwards.
  4. Note whether equipment was kept, and where — it is the first thing anyone asks for.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Does this replace an official report?

No. What must be reported, to whom and within what time differ by country and by the kind of premises or work, and this form names no scheme. Check what applies where you are and keep this alongside whatever that requires.

Why is there so much space for the account?

Because a two-line summary written from memory is worth very little, and a full account written on the day is worth a great deal. Let the person affected read it back before anyone signs.
