What this sheet is for

Leave a visit record that stands on its own. It shows what was read, what was done about it, and what the customer was told before the van pulled away.

Who it is for

Heating engineers and gas, oil and solid fuel service technicians, and the small firms whose engineers fill in a sheet at every service.

How to use it

  1. Identify the appliance from its data plate, not from what the customer calls it.
  2. Record each reading as found and as left, with the handbook's figure beside it and where you took it.
  3. Tick the checks as they are done; cross through any that do not apply to this appliance or fuel.
  4. List parts fitted, record what state the appliance was left in and any notice issued, then have the customer sign.

What is on the sheet

One page, top to bottom:

Printed under the title: One service visit to one appliance: readings as found and as left, the checks made, the parts fitted and what was said.

Readings, not limits. Reading columns are blank on purpose: the limits are the manufacturer's for that appliance and are compared on the certificate or printout your scheme has you issue. Write the handbook's figure in its column so the comparison is on the page.

Write-on lines: Customer · Address · Date · Engineer · Appliance make & model · Serial / GC no. · Fuel · Location · Age, if known

Readings — as found on arrival, as left on leaving

Table, 4 rows: Reading · Handbook figure · As found · As left · Taken where / how

Checks made — tick when done; cross through if not applicable

  • Flue and terminal visually checked along the route
  • Seals, gaskets and case fixings checked
  • Condensate trap and pipe cleaned and checked
  • Burner, exchanger and fan cleaned per handbook
  • Safety devices operated and proved
  • Controls, thermostat, timer and ventilation checked

Write-on lines: Parts fitted (part, part no., qty) · Appliance left · Warning notice issued? · Certificate / report no. · Next service: customer's or scheme's date · Customer told

Signature strip: Customer signature · Print name · Engineer

Questions

Why are there no acceptable ranges printed against the readings?

Because they belong to the appliance, not to a form: the manufacturer sets them and your scheme's certificate is where they are compared. The sheet asks you to copy the handbook's figure into its column so the comparison is visible on the page without pretending one figure fits every boiler.

How is this different from the homeowner's boiler service record?

That one is the customer's list of when a service happened and who came. This is the engineer's sheet for the visit itself — readings, checks, parts and outcome — and it stays with the engineer's job file.
