What this sheet is for

Give the customer, the monitoring centre and whoever services the system next a single page that says exactly what is fitted and where.

Who it is for

Intruder alarm installers fitting or taking over a domestic or small commercial system.

How to use it

  1. Fill in the panel, signalling path, monitoring reference and power supply from what was actually fitted.
  2. List every device against its zone number, with a location the customer would recognise.
  3. Tick each device as it is walk-tested with the customer watching.
  4. Work through the handover list, noting how many user codes were issued and to whom — never the codes themselves.
  5. Both sign, and leave a copy with the user guide.

What is on the sheet

One page, top to bottom:

Printed under the title: What went in, where each device is, how the system calls out and what the customer can now do. Grade, type and any maintenance interval are whatever your scheme and the customer's insurer define.

Write-on lines: Customer · Address · Date · Installer / company · Scheme reg. no. · Panel make & model · Grade / type · Signalling / comms · Monitoring centre & ref · Power & battery · Panel location

Devices fitted — one row per zone or device

Table, 7 rows: Zone · Device · Location · Wired / RF · Zone type · Tested · Notes

Handover — with the customer watching

  • Every device walk-tested with the customer
  • Full set, part set and unset demonstrated
  • User codes issued: how many, to whom (not the codes)
  • App or keyfobs set up and shown working
  • Monitoring centre test call confirmed
  • Keyholder details taken; user guide left

Signature strip: Installer · Customer · Date

Questions

Where do I write the grade and the maintenance interval?

In the blanks provided, using whatever your scheme, the customer's insurer and the rules where you work say applies. The sheet does not suggest a grade or an interval because those are yours to decide and to answer for.

Should the user codes go on this sheet?

No. Record how many codes were issued and who holds them, so a later engineer knows what exists. The codes themselves stay with the users.
