What this sheet is for
Hold the whole installation on one page the customer keeps, so the warranty, the commissioning readings and what they were shown are never in dispute.
Who it is for
Heating engineers installing a domestic boiler, and the customers who keep the record with the manual.
How to use it
- Fill in the appliance details from the data plate, and note what was removed.
- Tick the system work done — flush, inhibitor, filter, controls — with the product used.
- Take the commissioning readings your manual lists, and write the manufacturer's figure beside each one.
- Demonstrate the controls, pressure top-up and reset to the customer and tick the handover list.
- Both sign; the customer keeps it with the manual and the benchmark or logbook.

Questions
Why does the sheet not list the readings a boiler needs?
Because the list, and the acceptable figures, come from the manual for that exact model and from your registration scheme. The readings table gives you blank rows and a column for the maker's figure so the comparison is written down, not remembered.
Does this replace the manufacturer's benchmark or commissioning checklist?
No. If the manufacturer or your scheme provides one, that is the one that counts for the warranty. This is the installer's own record of the whole job, kept alongside it.
