A free printable review form — last period's objectives against what happened, what went well and what was harder, then what is agreed next.
2 pages. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.

Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Hold a yearly review that is about the work rather than about a score, with the employee's own comments recorded on the form.
Small employers, charities, practices and any manager who has been asked to do appraisals without a system to do them in.
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Because a review that reduces a year to a number tends to get argued about rather than acted on, and any scale worth using is one your organisation chose deliberately. If you use one, write it in.

See at a glance who has been trained in what and which squares are blank, across a whole team on a single page.
Record a supervision session so both people agree afterwards what was discussed and what was agreed.
We are building this library out steadily. Leave an address and we will send the new sheets as they are finished — nothing else, and no more than one email a week.
Sign-up is not live yet — we have not connected an email provider, so this form is switched off rather than quietly collecting addresses.
