What this sheet is for

Record a supervision session so both people agree afterwards what was discussed and what was agreed.

Who it is for

Care, support, youth work, counselling services, nurseries and any sector where supervision is part of the job.

How to use it

  1. Agree the agenda at the start, including anything the supervisee wants to raise.
  2. Write what was said, not a summary written afterwards from memory.
  3. Record wellbeing honestly; it is a section, not a formality.
  4. Both sign, and both keep a copy.
  5. Set the date of the next session before you finish.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

How often should supervision happen?

Many sectors have their own requirements and some are set by a regulator. This sheet sets no frequency and no format — follow whatever applies to your service.
