What this sheet is for

Keep one page per person showing what leave was requested, approved and actually taken, with the balance visible to both sides.

Who it is for

Small employers, family businesses, clubs with paid staff, and managers running a team without an HR system.

How to use it

  1. Write the year's entitlement in at the top from the contract or local rules.
  2. Record the date the request was made as well as the dates requested.
  3. Update the balance as leave is taken, not as it is booked.
  4. Agree the closing balance in writing at the end of the leave year.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

How much leave is somebody entitled to?

That depends entirely on where you are, the contract and often the sector, and it is not something a printable can answer. The sheet has a blank line for the figure that applies to you.
