What this sheet is for

Record days missed and time lost the same way for everybody, factually, without the sheet reaching any conclusion about it.

Who it is for

Small employers and managers who need a consistent record rather than a memory of who is often off.

How to use it

  1. Record it the same way for every person. Consistency is the point.
  2. Write what was said, not what you inferred.
  3. Note when the return-to-work conversation happened.
  4. Keep health information to what you actually need, and store it securely.

What is on the sheet

One page, top to bottom:

Printed under the title: A factual record of days missed and time lost, kept the same way for everybody.

A record, not a procedure. This records what happened and nothing else. It sets no trigger point, applies no policy and reaches no conclusion — those belong to your own procedures and, in most countries, to rules about how health information may be held. Entitlements, rates, records you must keep and how long you must keep them differ by country and often by sector. This sheet states none of them and is not employment advice — check what applies where you are.

Write-on lines: Name · Team or site · Period

Table, 11 rows: Date · Type · Full or part day · Hours lost · Notified by, and when · Reason given · Return-to-work conversation held on · Recorded by

Questions

Why is there no trigger point or absence policy on the sheet?

Because policies are yours to set and, in most countries, health information is subject to specific rules about what may be recorded and who may see it. This sheet records what happened and applies nothing to it.
