What this sheet is for
Record that somebody was actually watched doing the task, rather than that they attended a course about it.
Who it is for
Workshops, sites, farms, care services, kitchens and anyone signing off that a person is safe to do a job on their own.
How to use it
- Say where your criteria come from — a procedure, a handbook, a manufacturer.
- Watch the real task, not a description of it.
- Write what you saw, including what was done well.
- Record what further training or supervision is needed, if any.
- Set a reassessment date and put it in the training matrix.

Watching, not attending. Training is not competence. This sheet records an observation of the real task — who watched, what they saw, and what happens next. It sets no standard: the criteria come from your own procedures or the equipment manufacturer.

Does this certify anybody?
No. It records one assessor's observation on one day against criteria you supplied. Where a formal ticket or certificate is required, this does not replace it.
