The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.

The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.

Who this is for

A dated line for every day of an apprenticeship or training programme. Record the hours spent on the job and off it, what was learned, and who signed it off; the running totals build as the weeks go by.

Apprentices, trainees and anyone on a work-based course who has to show their provider or assessor how the hours were spent — and the employers and mentors who countersign the record.

What this helps you track

  • The hours worked on the job and the hours spent on training away from it, day by day
  • Running totals of both, so the position is known without adding up
  • What was learned or practised on each day, in enough detail to refer back to
  • Who saw it and signed it, so the record stands up when it is checked

How to use it

  1. Write your name, the employer, the training provider and the programme or standard you are on at the top, with the start date.
  2. Add a row for each day or shift: the date, the hours on the job and the hours off the job as your provider defines them.
  3. Write what you did or learned — the task, the skill, the unit — specifically enough to match it to your programme later.
  4. Ask your supervisor, mentor or assessor to sign the row, or write their name if they sign a batch at the end of the week.
  5. The running totals are worked out in the browser version and the workbook; on paper, add each row to the one above.
  6. Compare the totals with whatever your provider expects at each review, and keep every sheet until the programme is signed off.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

What counts as off-the-job hours?

Whatever your training provider and the rules where you are say counts. Definitions differ between countries, schemes and providers, so record the split as your provider has explained it to you rather than as any general rule.

How many hours do I need?

The sheet does not say, because it depends on your programme, your contract and the scheme's rules. Ask your provider, write the figure in the notes if you like, and use the running totals to see where you are against it.

Does it need to be signed every day?

That is between you and your provider. There is a signed-by column on each row; some people fill it daily, others get a week signed at a time.

Can I use it for a traineeship or a work placement?

Yes. Any programme that mixes doing the work with learning about it fits the columns — call the two kinds of hours whatever your scheme calls them.
