A free printable tail lift check sheet: a week of daily pre-use checks with the platform out, defects reported, and the lift's examination dates beside them.

The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Show that the tail lift was looked at every day it was used, not only at its examination. A worn roll stop or a weeping ram is found at the depot, before a cage goes over the edge.
Delivery drivers, operators and transport managers running vans and lorries fitted with a tail lift.
One page, top to bottom:
Printed under the title: One week of the operator's check on one tail lift, made each day it is used, with the platform folded out, before the first load.
The plate and the report set the figures. No rated load or examination interval is printed: copy the rated load from the plate on the lift and the dates from its last thorough examination report. A lift with a safety defect is not used until it has been put right.
Write-on lines: Vehicle registration · Lift make and model · Lift serial no. · Week beginning · Rated load (from the plate) · Last thorough examination · Next due · Report copy in cab? Y / N
Also printed: Check · Mon · Tue · Wed · Thu · Fri · Sat · Sun · Platform: no cracks, bends or damage · Platform surface grips, no oil or ice · Roll stops and flaps work and lock · Gates or side rails fitted and secure · No hydraulic leaks at rams or pump · Hoses and cables not chafed or kinked · Controls marked, stop when released · Emergency stop or isolator works · Warning flags and lights work · Rated load marking legible · Full up and down cycle, smooth and level · Stowage locks engage for travel · Defect found? Y / N · Initials
Table, 2 rows: Day · Defect found · Reported to · When · Out of use? · Put right on · By
Because the rated load belongs to that lift and is on its own plate, and how often it needs a thorough examination depends on the rules where you are and on the examiner's report. The sheet gives each a box, so the figure is copied from the source rather than from us.
No. The daily check is the operator's look before use; the thorough examination is a separate, deeper inspection by a competent person. The sheet keeps the date of the last one and the next one due in front of the driver.
Do the same walk-around in the same order every day and leave a record of what was found, who it was reported to and what happened next.
Get a fault from the driver's walk-round to the workshop in writing, with a closed loop showing it was fixed.
Record the check made before the first lift each day, crane and accessories together. A day ends in a plain safe or not safe, so nobody has to guess from a row of ticks.
Keep track of what lifting gear you hold, what each item is marked at, and when each was last examined.
Show, for one load, that the restraints used were the ones specified, were in fit condition and were checked again after the first miles and at each stop.
