What this sheet is for

Show that a new driver was actually inducted, item by item, rather than handed the keys and a phone number.

Who it is for

Small operators, owner-drivers taking on a second driver, and anyone responsible for putting a new driver into a company vehicle.

How to use it

  1. Fill in the driver, role and vehicle types before the first day.
  2. Work through the paperwork, policy and vehicle sections with the driver, ticking each item as it is covered.
  3. Add your own operation's items in the blank lines.
  4. Do the assessment drive and note anything agreed as follow-up training.
  5. Both sign, and file it with the driver's licence check.

What is on the sheet

2 pages, in this order:

Page 1

Printed under the title: A new driver's first days with an operator, ticked off item by item: the paperwork seen, the policies explained, the vehicle walked round and the drive assessed, signed by both sides.

Cover what applies to your operation. Which documents an operator has to see and what training a driver has to hold depend on the vehicle, the work and the rules where you are. This sheet is a record that each item was covered — add your own operator's requirements in the blank rows and strike out what does not apply.

Write-on lines: Driver · Start date · Operator / depot · Inducted by · Role · Vehicle type(s) to be driven · Vehicle reg. used · Assessment drive date

Paperwork seen and copied

  • Any professional qualification, badge or card the role needs seen and dated
  • Right-to-work and identity documents seen, per the operator's process
  • Medical or eyesight declaration completed, if the operator asks for one
  • Emergency contact and next of kin recorded

Policies explained

  • Hours, breaks and rest — the limits the operator works to, and how they are recorded
  • Walk-round check and defect reporting — what to check, and who to tell
  • Accident and breakdown procedure — who to call, what to photograph, what to say
  • Phone, drink, drugs and smoking policy while on duty
  • Fuel cards, tolls, fines and who pays what
  • Uniform, PPE and customer-facing conduct

Vehicle familiarisation

  • Walk-round with a supervisor: lights, tyres, mirrors, load area, coupling
  • Controls, warning lights and where the handbook and paperwork live
  • Tail lift, ramp, restraint points or other fitted equipment demonstrated
  • Telematics, tracker, camera or tachograph — how to log in and out
  • Fuel type, AdBlue and how to fuel the vehicle on the operator's cards

Assessment drive

  • Assessment drive completed with the vehicle type to be used
  • Reversing and manoeuvring in a confined space observed
  • Load security and delivery procedure walked through on a live job
  • Any further training or shadowing agreed, and by when

Ruled lines: Operator's own items — add what applies to this operation

Signature strip: Driver signature · Inductor signature · Date completed

Ruled lines: Notes — follow-up training agreed, anything outstanding

Questions

Why does the sheet not list which documents are required?

Because that depends on the vehicle, the kind of work and the rules where you are. The checklist names the common ones so nothing is forgotten; what is mandatory for your operation is for you to know and add.

What if a policy item does not apply?

Strike it through and initial it. A struck-out item shows it was considered, which is worth more than a blank.
