What this sheet is for

Get back what a trip cost you, in one claim with the receipts in order, rather than a shoebox of paper and an argument about the nights.

Who it is for

Employed and owner drivers who spend their own money on the road and claim it back from an operator, agency or client.

How to use it

  1. Fill in who the claim is to, the period and the rates they pay you.
  2. Add a row for every receipt or item as you get it — do not wait for the end of the week.
  3. Log each night away separately, with where you slept and what it cost.
  4. Total the claim and sign it; attach the receipts in the order they appear.
  5. Keep a copy until it is paid.

What is on the sheet

One page, top to bottom:

Printed under the title: What a trip cost you out of your own pocket — fuel, tolls, parking, meals and nights away — itemised against receipts for one claim. The rates are set by whoever you are claiming from, not by us: write theirs in.

Write-on lines: Driver · Claim ref. · Claim to · Period covered · Vehicle reg. · Job / contract · Mileage rate I am paid · Overnight rate I am paid

Expenses — one row per receipt or per item

Questions

Why are there no allowance or mileage rates on the sheet?

Because they are set by whoever you are claiming from, and by the tax rules where you are, and both change. The boxes at the top are for the rates you have been given; the sheet does the itemising, not the rate-setting.

What if I slept in the cab?

Log the night in the nights-away table with 'cab' in the lodging column and whatever allowance your operator pays for that, if any. The point is that the night is on record.
