What this sheet is for

Show, for one load, that the restraints used were the ones specified, were in fit condition and were checked again after the first miles and at each stop.

Who it is for

HGV, flatbed, curtainsider and van drivers carrying secured loads, and the transport office that has to answer when a load moves.

How to use it

  1. Describe the load and write the weight and restraint method you were given at loading.
  2. List every strap, chain, net or bar with its tag rating and its condition.
  3. Work through the before-moving-off checklist and sign the time you left.
  4. Re-check the restraints at your first stop and again later; note where and when.
  5. Take any damaged restraint out of use and write its ID and why at the foot.

What is on the sheet

One page, top to bottom:

Printed under the title: What is on the vehicle, what is holding it, the condition of each strap or chain, and a re-check at every stop — one sheet per load, kept with the delivery paperwork.

The rating comes from the tag, not from us. How many restraints, what rating and where they go comes from the load, the vehicle, the consignor, your operator, the rating tag and the rules where you drive. Write the figures you were given; this records that you used them.

Write-on lines: Vehicle / trailer · Driver · Date · Loaded at · Consignment ref · Load description · Weight (from consignor) · Restraint method specified by

Restraints in use — one line per strap, chain, net or bar

Table, 6 rows: Position · Type · ID / tag · Rating on tag · Condition · Fitted · Re-check 1 · Re-check 2

Before moving off

  • Load against the headboard, or blocked so it cannot slide
  • Weight sits where the plate and the consignor say
  • Edge protectors where a strap crosses a sharp edge
  • No twisted, knotted or repaired straps; hooks seated
  • Loose items and packaging cannot move or blow off
  • Curtains and doors latched; load not relying on them

Write-on lines: Any restraint taken out of use (ID, why) · Driver signature · Time departed

Questions

How many straps does my load need, and what rating?

That comes from the load, the vehicle, the consignor's instructions and the rules where you drive — and the rating of any strap is printed on its tag. The sheet has a blank for the method you were given and a column for each tag rating; it does not set either.

Why re-check twice?

Because loads settle and straps slacken once the vehicle has moved. Two re-check columns make the second look part of the job rather than an afterthought.
