What this sheet is for

Get a fault from the driver's walk-round to the workshop in writing, with a closed loop showing it was fixed.

Who it is for

Drivers of vans, trucks, buses, taxis and any vehicle where a fault has to be reported to someone else before it is fixed.

How to use it

  1. Fill in the vehicle, date, time and odometer as soon as the fault is found.
  2. Describe the defect and where on the vehicle it is, in enough detail to find it.
  3. Note where it was found — walk-round, on the road, at a drop.
  4. Hand the report to your operator or workshop; they complete the rectified line.
  5. Keep the page once all three reports are closed.

What is on the sheet

One page, top to bottom:

Printed under the title: Three tear-off style reports on one page: a fault found on a walk-round or during the day, written up with enough detail for the workshop to find it, and a line for them to say what they did about it.

You report; someone else decides. Whether a defect stops the vehicle going out is your operator's decision under the rules where you are — this pad records what you found and hands it on. Write 'reported as unfit' in the status box only if that is what you told your operator.

Questions

Who decides whether the vehicle can still go out?

Your operator, applying the rules where you are. The sheet has a vehicle status box for what was decided, not a printed rule for deciding it. If you were told not to drive it, write that.

Why three to a page?

Most days have no defects and a bad week has a few. Three to a page keeps the pad small while giving the workshop enough room to write what they did.
