A free printable lorry loader crane check sheet: a week of pre-use checks on the crane and its slings, chains and shackles, each day ending safe or not safe.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.

The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Record the check made before the first lift each day, crane and accessories together. A day ends in a plain safe or not safe, so nobody has to guess from a row of ticks.
Lorry loader crane operators, drivers of crane-fitted vehicles and the managers who hold the examination records.
One page, top to bottom:
Printed under the title: One week of checks on one lorry loader crane and its lifting accessories, made before the first lift of each day.
The chart and the reports set the figures. No capacity, test load or examination interval is printed: the crane's rated capacity chart gives its limits and the examination reports give the dates. Do not lift with a crane or accessory that is not safe to use.

The crane's rated capacity chart gives its limits for that crane and that set-up, and a printed figure could only be wrong for yours. The sheet asks you to check that the chart is there and legible instead.
The daily look is on this sheet. For each accessory's identity mark and its thorough examination history, use a lifting equipment register alongside it.
Keep track of what lifting gear you hold, what each item is marked at, and when each was last examined.
Show that the tail lift was looked at every day it was used, not only at its examination. A worn roll stop or a weeping ram is found at the depot, before a cage goes over the edge.
Do the same walk-around in the same order every day and leave a record of what was found, who it was reported to and what happened next.
Get a fault from the driver's walk-round to the workshop in writing, with a closed loop showing it was fixed.
