Show for every load when the trailer or lorry was cleansed and disinfected, where and with what. It also records who checked it before the next animals went on.

One page, top to bottom:
Printed under the title: One trailer or lorry, one line per load: what was carried and between where, when it was unloaded, when and where it was cleansed and disinfected, with what, and the check before it was loaded again.
Your rules, your label. None of these is printed: write yours in the boxes below and check the rules locally.
Write-on lines: Vehicle or trailer · Registration or ID · Owner or haulier · Year · Disinfectant in use · Dilution from the label · Time limit you work to, after unloading
Also printed: Journey · Cleansed and disinfected
Table, 11 rows: Date · Animals · From · To · Unloaded · When · Where · Disinfectant · By · Checked
Write-on lines: Any load not cleansed straight after unloading: why, and where it was done · Reviewed by

Keep the visitor record that matters when disease is traced, and make the boot dip a thing that is recorded rather than assumed.
Record cleaning and disinfection properly, with the contact time and rest period that decide whether it achieved anything.
Run the same walk-around every time, in the same order, so nothing gets skipped because you were in a hurry.
Follow a batch of bought-in or returning animals through isolation until they join the rest. The tests, the daily checks and the vet's agreement are all on one record.
