What this sheet is for

Keep your own copy of every movement on and off, with the reference numbers that make it possible to answer a question about a load six months afterwards.

Who it is for

Smallholders and farms moving animals to market, to grazing, to slaughter or between holdings.

How to use it

  1. Record the movement on the day, then note the date you reported it.
  2. Write both holding numbers in full, yours and theirs.
  3. Keep the document reference — it is the thread that ties this to the official record.
  4. Note when your retention period ends so sheets are not thrown out early.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Does filling this in count as notifying the authorities?

No. Movements usually have to be reported within a set time, by a specified method, and the requirements differ by country and species. This is your own copy of what happened. Check what you must report and to whom.
