The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Because they are on the label, they differ by product and by country, and they change. Using a figure from a website instead of the container is how applications go wrong. This sheet records what the label said and what you did.
In many countries, for many products, yes — and there are often separate requirements for the equipment. What applies depends on where you are, what you are applying and why. Check locally.
