Keep the checks on an oxy-fuel set in one place with the parts that make it safe. After a flashback, the sheet shows what was checked and replaced before it was lit again.
Fabricators, heating engineers, garages, plumbers and anyone using an oxy-acetylene or oxy-propane set for cutting, welding, brazing or heating.
2 pages, in this order:
Printed under the title: One oxy-fuel set: the regulators, flashback arresters and hoses on each line recorded once, then every check of the set, every flashback or backfire and every part replaced.
The makers set the pressures and lives. No pressures, hose lengths or replacement dates are printed. Gas pressures come from the torch and nozzle maker's chart; the life of regulators, arresters and hoses comes from their makers and your supplier. After any flashback, take the set out of use and check the torch, hoses, regulators and arresters before it is lit again.

Because working pressures depend on the torch and nozzle, and the life of a regulator, arrester or hose is set by its maker. Use their charts and dates, and write them on the sheet where they apply.
Not with tape or a patch. Cut back past the damage and refit proper hose fittings, or replace the hose, and record what was done in the parts table.

Control work that makes heat, flame or sparks, and record the after-check that catches the fire that starts once everyone has gone home.
Catch a cut lead, a hot connection or a leaking gas hose before the arc is struck. Faults and servicing go on the back, so the set's history stays with it.
Keep a dated record of monthly visual checks, and separate them clearly from the professional servicing that premises usually also need.
