What this sheet is for
Keep a per-machine service history for fixed workshop equipment, so the consumable part numbers and the last service date live with the machine.
Who it is for
Workshops, garages, joineries, farm sheds, community workshops and anyone responsible for machines that other people use.
How to use it
- Print one sheet per machine and fill in the specification block once.
- Record hours where the machine has an hour meter and dates where it does not.
- Log the parts actually fitted, with part numbers — that block is the reason to keep the sheet.
- Hang it near the machine or keep the set together in one folder.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
What service interval should I use?
The one in your own handbook for that machine. Intervals vary enormously between makes, duty cycles and environments, so the sheet leaves the interval blank for you to write in rather than printing a figure that would be wrong for most readers.
Is this a maintenance certificate?
No. It is a private record of what was done and who did it. Where equipment is used at work, the inspection and record-keeping duties that apply to it are set by the country you are in — check those separately.
