Turn a pile of unexplained adjustments into a list with a reason against each one, so the next stock count has fewer surprises.
Stores and warehouse staff, stock controllers and small business owners who correct their own stock figures and want a record of why.
One page, top to bottom:
Printed under the title: Every time the system quantity is changed by hand — what it said, what was actually there, the difference, why, and who agreed to it — so the adjustments themselves can be counted, not just the stock.
Your reason codes. Reason codes are yours: use the ones your stock system already has so this sheet and the system agree. An adjustment with no reason written against it is the one that gets asked about at the next count. Who may approve an adjustment is decided where you work.
Write-on lines: Site / stores · Period · Stock system · Log kept by

Because most systems record that a figure changed, not why in words anyone will read later, and not who was asked before it was changed. This log is the reason and the approval, kept where the next stock count can find it.
That is set where you work. The sheet has an approved-by column on every row and no threshold printed, so you can apply your own rule.
Count stock area by area and see the variance beside the count, so a stock take produces a number you can act on rather than a vague reassurance.
Keep suspect, damaged or recalled stock out of the pick until someone has decided what happens to it, and see at once which holds are still open.
Find out whether picking errors are one picker, one aisle or one kind of mistake, before the customer complaints tell you.
Stop returns going straight back on the shelf unchecked, and keep the customer's story and the bench's findings side by side.
