What this sheet is for

Check the agency's invoice against a page written at the gate on the day, rather than against a rota that says who was meant to come.

Who it is for

Warehouse supervisors, goods-in and despatch team leaders and anyone who signs off agency hours at a depot, distribution centre or stores.

How to use it

  1. Fill in the site, week and shift once at the top.
  2. Each worker prints their name, agency and badge or reference as they sign in.
  3. They write the time in and out and the break taken; the supervisor works out the paid hours and initials the row.
  4. Note the area or role, so an invoice query can be matched to the job they did.
  5. File the sheet with the week's agency invoice.

What is on the sheet

One page, top to bottom:

Printed under the title: Who from which agency was on site, when they arrived and left, where they worked and what they signed for — one line per person per shift, so the agency's invoice can be checked against a page you wrote at the time.

Hours recorded, not hours allowed. No rates, no break entitlements and no hours limits are printed here. The hours an agency worker can work, the breaks they take and what each hour costs are set by your agency agreement and the rules where you are — this sheet records what actually happened, in ink, at the gate.

Write-on lines: Site / depot · Week beginning · Shift · Supervisor

Sign-in — the worker signs in and out; the supervisor initials the hours

Table, 12 rows: Date · Name · Agency · Badge / ref · In · Out · Break · Hours · Area / role · Sig. · Sup.

Questions

Why does the sheet not say how many hours a worker can do?

Because that is set by your agreement with the agency and the rules where you are, and it is not the same for every site or every worker. The sheet records what was worked; whether that was allowed is a question for the agreement, not the page.

Should the worker or the supervisor fill in the hours?

The worker writes their own times in and out and signs; the supervisor fills in the paid hours from those and initials. Two hands on the row is what makes it worth keeping.
