Say, for any driver on any truck, on what basis they are allowed to be on it — and show the date that authorisation was given, renewed or taken away.
Warehouse managers, site supervisors and anyone who signs an operator on to a forklift, reach truck, pallet truck or other lift truck at a site.
One page, top to bottom:
Printed under the title: Who on this site is authorised to drive which trucks, on the strength of what training evidence, from when, by whom — and when that authorisation was withdrawn or renewed. One row per operator per truck type.
Your scheme's terms, your site's signature. This sheet states no training requirement and no refresher interval. What evidence counts, which truck types need separate authorisation and when a refresher falls due are decided by your own training provider, your insurer and the rules where you are — the dates in the last columns are theirs, written in by you.
Write-on lines: Site · Record kept by · Sheet opened · Sheet closed
Table, 12 rows: Operator · Truck type · Evidence / ref · Trained · Familiarised · Authorised by · From · Refresher · Withdrawn
Because that is set by your training provider, your insurer and the rules where you are, and it differs by truck type and by country. The sheet records the evidence you accepted and the dates your scheme gave; it does not decide whether they were enough.
Trained is the date on the certificate or course. Familiarised is the date the operator was shown this site's trucks, aisles and rules. They are often weeks apart, and an authorisation usually waits for the second.
Give the operator a fixed order to walk the truck in, and give the supervisor a signed page that says what was checked and what was found before the first lift.
Keep a week of daily walk-around checks on a single sheet that lives with the machine, so the record is made where the machine is rather than in an office.
See at a glance who has been trained in what and which squares are blank, across a whole team on a single page.
Keep track of what lifting gear you hold, what each item is marked at, and when each was last examined.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee.
