Site managers, supervisors and welfare cabin providers on building sites, groundworks and any job with cabins on hire.
One page, top to bottom:
Printed under the title: One walk-round of a site's welfare cabins — toilets, washing, drying, canteen, water, heat and power — with the faults found and who is fixing them.
Your site's standard, checked. How many toilets a site needs and how often cabins are cleaned come from the rules where you work and your own arrangements — write yours at the top. This sheet is the walk-round: what was found and what was done.
Write-on lines: Site · Date & time · Checked by · People on site today · Cabins (type & how many) · Cabin supplier / hire ref · Cleaned by & how often (yours) · Water supply
Table, 2 rows: Item · Fault · Reported to · Fixed on · By
Write-on lines: Checked by (signature) · Print name · Seen by site manager · Date
Because those numbers come from the rules where you work, the size of the workforce and the arrangement you have with your cabin supplier. The sheet has a box for your own cleaning frequency and checks that what you have set up is actually working.
As often as your own site arrangements say, and that is your call, not this sheet's. One sheet is one check, dated and signed.

Know who is on the premises at any moment, and be able to take that answer to the assembly point as a single sheet of paper.
Settle the questions a scaffold hire always raises — who asked for the extra lift, when off-hire was called and what went missing — from a signed record rather than a phone call.
Record a short on-site briefing and, more usefully, what the people listening raised in response to it.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
