What this sheet is for

Record why you let someone in and how many keys now exist, on one sheet. Those are the two things a locksmith is asked about later, and the customer signs for both.

Who it is for

Locksmiths and emergency entry specialists, and property managers who want a record of every lock changed and every key issued.

How to use it

  1. Write the reason for the call, who called and what proof of entitlement you were shown before starting.
  2. Take one line per door or lock: what was there, what you did, how you gained entry and any damage.
  3. Count the keys cut and handed over per lock, note any key reference and what happened to the old keys.
  4. Test every lock from both sides, then have the customer sign for the keys.

What is on the sheet

One page, top to bottom:

Printed under the title: One call-out: why you were called, how entitlement was checked, each lock worked on and every key handed over.

Entry checked, keys counted. Whether the caller is entitled to enter is your judgement under your trade body's guidance and the rules where you work; the first line records what you were shown. Keys are counted per lock and signed for.

Write-on lines: Customer · Address · Date & time · Locksmith · Reason for call-out · Called by · Proof of entitlement seen · Satisfied? · Police / insurer ref

Doors and locks — one line per door or lock worked on

Table, 3 rows: Door · Existing lock · Action · Method of entry · Lock fitted · Damage

Keys — one line per lock — cut, handed over, and what happened to the old ones

Table, 3 rows: Lock / door · Keys cut · Handed over · Key ref / code · Restricted? · Old keys

Write-on lines: Locks tested both sides? · Door closes & locks freely? · Customer shown mechanism? · Swarf & old parts cleared?

Signature strip: Customer signature — keys received · Print name · Locksmith

Questions

What proof of entitlement should I ask for?

That is your decision, made under your trade body's guidance and the rules where you work — the sheet does not set a standard. It records what you were shown and that you were satisfied, which is what you will need if the question is ever asked.

Why count keys per lock rather than in total?

Because a key count only means something against the lock it opens. Four keys across two locks tells nobody anything; two for the front and two for the back tells the customer, the landlord and the next locksmith exactly what exists.
