What this sheet is for

Get the customer's signature that the work is done while you are still standing there, with anything outstanding written down rather than remembered differently.

Who it is for

Trades, installers, service engineers and small contractors invoicing on completion.

How to use it

  1. Fill it in on site, with the customer, before you pack up.
  2. List outstanding items honestly — a signed form with two snags on it is far stronger than one claiming perfection.
  3. Tick the handover items as you actually hand them over.
  4. Photograph the completed work and note that you did.
  5. Leave the customer a copy.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Does a signature here mean they have to pay?

It means they agreed on the day that the work was done and what was left outstanding. Payment terms come from your contract or your invoice. But most payment arguments are really arguments about whether the work was finished, and this is the document that settles that one.
