What this sheet is for

Tie every job to the circuits it touched and the certificate it produced, so the paperwork trail is complete without test results being copied onto a card.

Who it is for

Electricians and small electrical firms doing domestic and light commercial work, and the office that files the cards afterwards.

How to use it

  1. Write the work requested or the fault reported in the customer's words.
  2. Log each circuit or location touched, who proved it dead, what was done and the device fitted.
  3. Record the certificate type and number, who it was notified to, and have the customer sign.

What is on the sheet

One page, top to bottom:

Printed under the title: One electrical job on one card: what was asked for, which circuits were touched, what went in and which certificate went out.

Results go on the certificate. Test results are not recorded here — they belong on the certificate or report your scheme has you issue; the card records which one went out and its number. If none was needed under your scheme, say so in the box.

Questions

Why is there nowhere to write test results?

Because test results belong on the certificate or report your scheme has you issue for that kind of work, in the layout the scheme sets. Copying them onto a job card invites a mismatch between the two. The card records which certificate was issued and its number, which is what the office needs.

What if the job did not need a certificate?

Write that in the certificate box, with a word on why, rather than leaving it empty. A blank looks like a missed one; a note says a decision was made. Whether it needed one is set by your scheme and the rules where you work.
