What this sheet is for

Record the design decisions for one charge point beside the unit they apply to, so the installation can be understood by whoever looks at it next.

Who it is for

Electricians installing home and workplace charge points, and the customer who wants to know what was fitted and who was told.

How to use it

  1. Record the make, model, serial and rating as marked on the unit.
  2. Write in where the supply comes from, the cable, route, protective device and earthing arrangement you designed.
  3. Work through the handover with the customer — a charge started on their own vehicle is the point that matters.
  4. Enter the certificate, who was notified and any scheme reference, then sign.

What is on the sheet

One page, top to bottom:

Printed under the title: One charge point: the unit, its supply and route, how it was protected and earthed, and what the customer was shown.

Your design, recorded. Cable size, protective device, earthing and fault protection are your design decisions for this installation, made under your scheme's rules and the manufacturer's instructions. Each has a blank so the decision is written down. Test results go on the certificate, not here.

Write-on lines: Customer · Address · Date · Installer · Charge point make & model · Serial no. · Rating as marked · Tethered / socket · Mounted on

Supply and route

Write-on lines: Supply taken from · Cable type & size · Route & length · Containment · Protective device (make, type, rating) · Earthing arrangement · PEN / open-neutral protection · Load management?

Commissioning and handover — tick when done, cross through if not applicable

  • Charge started with the customer's own vehicle
  • App, card or account set up with the customer
  • Schedule or off-peak setting explained, if it has one
  • Stopping a charge and the lights explained
  • Guarantee registered in the customer's name
  • Cable route and isolation point pointed out

Certificate and notifications

Write-on lines: Certificate type & no. · Network operator notified · Scheme / grant ref · Photos taken?

Ruled lines: Notes — anything about the existing installation the customer was told about

Signature strip: Customer signature · Print name · Installer

Questions

Why does the sheet not say what cable or protection to use?

Because those are design decisions for the installation in front of you, made under your scheme's rules and the manufacturer's instructions for that unit. The sheet gives each decision a blank so it is written down, not a value it cannot know.

Where do the test results go?

On the certificate your scheme has you issue for the work. This record notes the certificate type and number, so the two documents point at each other.
