What this sheet is for

Leave a clear record of a blockage call-out, so a repeat visit to the same run is recognised as a repeat and not treated as new.

Who it is for

Drainage engineers, jetting operatives and plumbers who clear blocked drains, and anyone running a small drainage business from a van.

How to use it

  1. Write down the problem as the customer reported it, and where the water was backing up.
  2. Take one line per run worked — access point, what was found, the cause and how it was cleared.
  3. Note the machine, nozzle, hose run and where the water came from.
  4. Record whether this run has blocked before and whether you suggested a camera survey, then have the customer sign.

What is on the sheet

One page, top to bottom:

Printed under the title: One jetting or rodding call-out: the run, the blockage, how it was cleared and what the customer was told.

Clearance, not survey. Clearing a drain is not surveying it. This sheet records that the run was cleared and what came out; if the camera went down afterwards, the findings belong on the Drain Survey and Findings Sheet. Note here only what you saw from the access point.

Questions

Why is the survey on a different sheet?

Clearing a drain and surveying it are different jobs with different outputs. This sheet says the run was cleared and what came out of it; a camera survey produces observations at distances, which need their own table. Keeping them apart means a clearance record is not mistaken for a condition report.
