What this sheet is for

Capture a CCTV or walk-through survey on site in a form a customer can read, with the coded version ready to be written up to the standard the job asks for.

Who it is for

Drainage surveyors and CCTV operators, and builders or home buyers who commission a survey and want the findings in one place.

How to use it

  1. Fill in the equipment, recording reference and the standard the survey is being done to.
  2. Log each run once: from and to, use, size, material, length and direction.
  3. Record every observation in the order the camera met it, with the distance from the start and a plain-words description beside the code.
  4. Suggest an action per observation and sign the sheet.

What is on the sheet

2 pages, in this order:

Page 1

Printed under the title: A CCTV or walk-through drain survey written up run by run — where each run starts and ends, what it is made of, and every defect or observation found along it with the distance it was found at.

Your scheme's codes, in your hand. No observation codes or condition grades are printed here. The coding system and the grading scale come from the standard the client asked the survey to be done to. Write the code you are working to in its column and its plain-language meaning beside it, so a customer can read this sheet without the manual.

Questions

Why are no observation codes or grades printed on the sheet?

Because the codes and the grading scale belong to the standard or scheme the survey is being done to, and there is more than one. The sheet asks which one you are working to and gives the code its own column; the plain-words column beside it is what the customer reads.
