Make the same nine checks at every tip point and leave a dated line to show it. The near misses on page two are the ones worth talking about before the next job.
Tipper lorry and tipping trailer drivers, and the transport managers and site operators who run tipping on their sites.
2 pages, in this order:
Printed under the title: One line for every tip: where it was, and the checks made before the body went up and after it came down. Tick each numbered check, or put X, do not tip, and say what was done in the notes.
The maker and the site set the limits. No slope, wind or clearance figure is printed. The vehicle and body makers set the limits for tipping, and each site sets its own rules for where and how: follow both, and ask the site controller when either is unclear.
Write-on lines: Driver · Vehicle registration · Body or trailer · Dates covered
Also printed: Reported to the site controller or banksman on arrival · Load spread evenly, not frozen or stuck in the body · Ground at the tip point level and firm · Tailgate release method agreed and understood · Overhead lines and structures clear of the raised body · Cab and body or trailer in line, parking brake on · Everyone not needed kept clear of the vehicle · Body fully lowered before moving off · Edge protection or wheel stop suitable, if tipping at an edge · Before tipping · After

Table, 4 rows: Date · What happened or was found · Who was told · When · Action taken

Because they belong to the vehicle and body maker and to the site, and they differ from one tipper and one site to the next. A printed figure could be wrong for your body, so the sheet asks you to follow the maker and the site controller instead.
Nine named columns would not fit across a portrait page at a size anyone could read. The numbered key keeps every check written out in full, and leaves room in the table for the site and the notes.
Do the same walk-around in the same order every day and leave a record of what was found, who it was reported to and what happened next.
Capture the things that nearly went wrong, in a form that asks what happened rather than who was at fault.
Get a fault from the driver's walk-round to the workshop in writing, with a closed loop showing it was fixed.
Show, for one load, that the restraints used were the ones specified, were in fit condition and were checked again after the first miles and at each stop.
