What this sheet is for

Capture the things that nearly went wrong, in a form that asks what happened rather than who was at fault.

Who it is for

Sites, workshops, farms, warehouses, kitchens, care services and any workplace that would rather find out early.

How to use it

  1. Make reporting easy and make it clear it is not a disciplinary process.
  2. Write what happened factually and what could have happened at worst.
  3. Agree actions with owners and dates, and close them out.
  4. Review near misses together — patterns show up across reports, not within one.
  5. Leave the reporter's name optional if that is what gets them filled in.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
