Fuel and MPG Log
See what a vehicle actually costs to run and spot a change in fuel consumption early, which is often the first sign that something mechanical is wrong.

A free printable driver fuel receipt sheet — date, station, odometer, litres, product, amount, how it was paid and the receipt number for every fill.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.
Print it and write on it
Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Reconcile a fuel card statement in one sitting instead of hunting for the receipt that matches a line, and see consumption from the odometer readings either side of each fill.
Drivers who fill up on an operator's fuel card, owner-drivers claiming fuel back, and the transport office that checks the statement against the paper.
One page, top to bottom:
Printed under the title: Every fill for one driver and one vehicle over a period — where, how much, what it cost, how it was paid and which receipt goes with it — so the fuel card statement reconciles first time.
The odometer is the point. Write the odometer at every fill, not just some of them. A line with a reading either side is a consumption figure; a line without one is only a receipt. The receipt number column is the link to the paper your operator or accountant wants.
Write-on lines: Driver · Vehicle reg · Fuel card (last digits) · Period
Table, 10 rows: Date · Site / station · Odometer · Litres · Product · Amount · Paid by · Receipt no. · Notes
Signature strip: Driver · Checked against statement by · Date
Because the figure that matters is yours, from your vehicle, your load and your roads, and it comes out of the odometer and litres columns once there are a few lines. Any target for it is your operator's to set, not this sheet's.
Whatever went in: diesel, petrol, AdBlue, oil, screenwash. A card statement lists them separately and so should the log, or the litres never match.

Capture the road costs that never reach the fuel log, so an expense claim can be assembled from a record rather than from a pocketful of tickets.
Decide the day once, on paper, before the engine starts — and keep the gate codes, closing times and awkward approaches where the next driver can find them.
Claim money spent on somebody else's behalf in a form that ties each line to a numbered receipt and gets signed off.
