What this sheet is for
Claim money spent on somebody else's behalf in a form that ties each line to a numbered receipt and gets signed off.
Who it is for
Small businesses, clubs, charities, committees, volunteers and anyone reimbursing staff or members.
How to use it
- Number the receipts and write the number in the row, then attach them in order.
- Use your organisation's own categories.
- Sign the declaration — it is what makes the claim a document rather than a note.
- Have someone other than the claimant approve it.
- Record the date it was paid, on the form.

No rates are printed here. What can be claimed, what evidence is needed and how mileage is treated are set by the organisation paying and by local tax rules. This form prints no rates and no rules; it records the claim.

Questions
Why is there no mileage rate on the form?
Because rates are set by the organisation paying and, in many countries, by a tax authority — and they change. Printing one would be wrong for most readers within a year. Write the rate you are using in the row.
