What this sheet is for

Get a defect reported in writing with a date on the report, the response and the completion, so it can be chased rather than remembered.

Who it is for

Landlords and tenants, village halls, churches, schools, offices, managing agents and anyone maintaining a building used by others.

How to use it

  1. Report it in writing even when you have also said it out loud.
  2. Say what is unsafe or unusable now — that is what sets the priority.
  3. Record whether it was made safe temporarily, and how.
  4. Take a photograph and note the reference.
  5. Do not close it until the work is actually complete.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
