The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
The record of a treatment visit and the visits that follow it. It names the pest and the evidence found, lists every location treated or monitored, and keeps the follow-up visits on the same sheet so the whole programme totals together.
Pest control technicians and small firms working for householders, landlords and commercial premises, who need a signed record per programme rather than per call.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Because what may be used, by whom, at what rate and with what record is set by the rules and the label where you are — none of which we can supply. Write what you used into your own treatment record.
The log on this site is the premises record: a rolling history of monitoring at one building. This is the contractor's job sheet for one programme — locations, visits, hours, charges and a customer signature.
That depends on the pest, the site and what you find, so the sheet gives you lines rather than a number. Book the next one before you leave and write the date on the sheet.
Yes, and usually should. It shows where points were placed, what was found and what they were asked to change — which matters if an inspection or a landlord asks later.
What you found at each item or area, in your own words.
What was inspected on the day, and by whom.
Who raised the order, when, and against which asset or location.
Printable sheets, workbooks and tools on the same subject.
Keep sightings, monitoring points and contractor visits in one dated place so a pattern shows up before an infestation does.
Get a defect reported in writing with a date on the report, the response and the completion, so it can be chased rather than remembered.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee.
