Gym Equipment Inventory and Value
Every item of equipment with its asset tag and serial number.
Inspections and safety checks · IC-119 · Record
Free gym equipment safety inspection record — frames, cables, pads and safety stops checked machine by machine, with faults counted. PDF, Excel and browser.
One page. The A4 and US Letter files are laid out identically, so neither is shrunk to fit.
Print it and write on it
Fillable PDF type and save
Spreadsheet formulas included
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
One line for every machine and rack on the gym floor. Four columns cover the frame and fixings, the cables and belts, the pads and grips and the safety features, with the fault in words and a tick to take a machine out of use.
Gym and leisure centre operators, personal trainers with their own studio, club and hotel gyms, school and workplace fitness rooms and anyone responsible for kit that members use unsupervised.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
No. A service record says what an engineer did to a machine and when it is booked in again. This is the safety round carried out by the people on site between services, and the two are usually kept side by side.
The sheet does not say. Frequency depends on how busy the gym is, what the maker's instructions tell you and what your own policy and insurer expect. Date each sheet and keep the run of them.
Yes. Give racks, benches, bars and dumbbell sets their own lines: collars, knurling, loose plates and damaged benches all belong on the record.
Add a line, date it and mark it as reported in the fault column. A record that only shows planned rounds misses the faults that actually get found.
Every item of equipment with its asset tag and serial number.
Every item of equipment on the site, with its own reference.
Which machines exist, where they are and which have been checked this round.
That tools and surfaces were dealt with between clients, not just at the end of the day.
Printable sheets, workbooks and tools on the same subject.
Track the usage measure that actually decides when something is replaced, instead of guessing from how old it looks.
Get a defect reported in writing with a date on the report, the response and the completion, so it can be chased rather than remembered.
Capture an incident properly on the day, in the affected person's own words, with the witness details and equipment notes that become impossible to gather later.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
