Keep sightings, monitoring points and contractor visits in one dated place so a pattern shows up before an infestation does.

Who it is for

How to use it

  1. Record every sighting, including ones staff mention in passing.
  2. Give monitoring points a reference and use it consistently.
  3. Note the evidence seen, not just the conclusion.
  4. Record proofing and housekeeping actions — they are what actually fix it.
  5. Keep contractor reports with this log.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why are no treatments or products named?

Because what may be used, by whom, and with what records differ by country and by premises type, and much of it is restricted to licensed operators. This is a record of what was seen and done.
