The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Because what may be used, by whom, and with what records differ by country and by premises type, and much of it is restricted to licensed operators. This is a record of what was seen and done.

Run an audit against your own questions, with evidence and actions recorded beside each one rather than in a separate note.
Keep a running index of waste leaving the premises and of the documents that came back with it, so a request for records is a filing question.
