Livestock keepers, smallholders and farm staff who buy in, store and dispose of veterinary medicines.
2 pages, in this order:
Printed under the title: Every veterinary medicine that comes onto the holding, with its batch, supplier and prescription; then the store and fridge checks, opened containers and anything disposed of.
The label and data sheet set the figures. This is the buying-in and storage side; what was given to which animal goes on an animal medicine record. No storage temperature, in-use life or disposal method is printed: each comes from the product's label or data sheet, your vet and the rules where you are, which also say how long these records must be kept.

The animal medicine record logs what was given to which animals, with the withdrawal date. This sheet covers the other half: medicines coming onto the holding, how they are stored, when containers were opened, and how anything left over was disposed of.
Because they differ between products and are set by each product's label or data sheet. The sheet leaves a blank for the range you keep to and a column for each container's discard-by date, copied from its own data sheet.
