See the true level of mastitis in the herd, mild and untreated cases included. Repeat cows and problem quarters then show up on paper instead of being half remembered.
Dairy farmers, herd managers and milkers in any size of herd, and smallholders milking a house cow or two.
2 pages, in this order:
Printed under the title: Every clinical case on one line, by cow and quarter, the mild and untreated ones included: the CMT or cell count result, the treatment as prescribed, the date her milk is clear and whether she has had it before.
Your scale, your vet's treatment. No cell count threshold, severity grade, treatment or withdrawal period is printed. Name the severity scale your farm and vet use, copy each treatment and its withdrawal from the prescription and the product label, and write down the mild cases left untreated: they are part of the herd's true count.

Because what counts as a high cell count, which cases are treated and with what, and how long milk is withheld all come from your vet, your milk buyer and the product label, and differ between farms and countries. The sheet records your figures beside each case instead.
Yes. A record that only holds treated cases makes the herd look better than it is and hides the cows that keep flaring up. The severity column lets you write them down as mild without suggesting they were treated.
The medicine record is the legal-style record of every medicine given to any animal. This sheet follows mastitis itself, quarter by quarter and cow by cow, including cases that got no medicine at all. Treated cases belong on both.

Record yields per animal daily, with the notes that turn an unexplained drop into an early warning.
Show the milk was cooled after every milking, with the time and initials of whoever read it. Anything outside the buyer's limits is written up with its cause and fix.
Change liners when your own herd's numbers say they are due, not when milking starts to slow. The rest of the rubberware and every machine test get their own record.
Hold the herd's detail in one legible place, so tags, parentage and movements can be read off a page instead of assembled from memory and text messages.
