Get the facts of a road or bridleway incident down on paper the same day. A clear account with a registration and footage is what any report will need.
Riders, carriage drivers and anyone leading a horse on roads, bridleways or other shared routes.
One page, top to bottom:
Printed under the title: The facts of a road or off-road incident or near miss, written the same day, ready for the police, an insurer or a riding body.
The same day. Write it down while it is fresh. Note the registration exactly as you saw it, and save camera footage from before and after, with the registration in view. Who you report to is your choice: the police, your insurer and riding bodies such as the BHS each keep their own records.
Write-on lines: Date · Time · Rider · Horse · Where: road or route, nearest landmark · Weather · Light
Write-on lines: Other party: vehicle type, colour, make · Registration · Did they stop?
Ruled lines: What happened, in order: what you did, what they did, what the horse did
Write-on lines: Injuries to the horse · Injuries to the rider · Witness: name and contact · Footage: saved where, covers before and after? · Police: date, reference · Insurer: date, reference · Riding body: date, reference
Signature strip: Written by · Signed · Date written
Yes. Riding bodies such as the BHS ask riders to report near misses as well as collisions, because they show where the danger is before anyone is hurt. The form works the same way for both.
No. It is your own record, written while the details are fresh, which you can then use to fill in whatever the police, your insurer or a riding body asks for. Keep it with the footage.

Capture the things that nearly went wrong, in a form that asks what happened rather than who was at fault.
Know which hat has been dropped or fallen in, and when it was taken out of use. Shared hats get passed round, and the register keeps track of each one.
Keep the veterinary history and the insurance position in one place, which matters when a claim depends on showing continuity.
