What this sheet is for
Keep a vaccination history with batch numbers, which is the record a competition venue, a livery yard or an insurer will ask to see.
Who it is for
Owners, competitors, livery yards and anyone whose horse travels.
How to use it
- Record the batch number from the vial — a passport entry without one gets queried.
- Note exactly which vaccine and against what.
- Have the vet sign, and treat the passport as the primary record.
- Diarise the next due date somewhere other than this sheet as well.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
What vaccination schedule should I follow?
None is printed. Requirements differ by country, discipline and governing body, and competition rules about intervals are strict — being a day out can invalidate an entry that was booked months ago. Your vet and your governing body set the schedule.
Does this replace the passport?
No. Where a passport is required it is the legal document and the vet's entry in it is what counts. This is a working copy that is easier to consult and harder to lose.
