What this sheet is for

Keep a running record of what a child has read and how it went, without attaching a level or an age to any of it.

Who it is for

Parents, teachers, tutors, teaching assistants, reading volunteers and libraries.

How to use it

  1. Fill it in straight after reading, in a line or two.
  2. Write what to work on next time — that is the column that helps.
  3. Record who they read with; the pattern matters.
  4. Note books abandoned as well as finished. That is useful information too.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why is there no reading level or age column?

Because levelling schemes differ by country and by school, and a level printed here would be somebody else's system. If your school uses one, write it in the comment column.
