What this sheet is for
Keep a running record of what a child has read and how it went, without attaching a level or an age to any of it.
Who it is for
Parents, teachers, tutors, teaching assistants, reading volunteers and libraries.
How to use it
- Fill it in straight after reading, in a line or two.
- Write what to work on next time — that is the column that helps.
- Record who they read with; the pattern matters.
- Note books abandoned as well as finished. That is useful information too.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
Why is there no reading level or age column?
Because levelling schemes differ by country and by school, and a level printed here would be somebody else's system. If your school uses one, write it in the comment column.
