What this sheet is for

Plan a trip on paper and record that your own requirements were met, then use the second page on the day for groups and head counts.

Who it is for

Teachers, youth group leaders, scouts and guides, sports clubs and anyone taking a group off site.

How to use it

  1. Establish what your setting and local rules require first, then use this to record it.
  2. Write the risk assessment for this trip and this group, not last year's.
  3. Agree the head count method and when it will be done, in advance.
  4. Give every adult a group list and a contact number.
  5. Keep the second page with you on the day, not in the office.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

That differs by country, by setting, by age and by activity, and often it is set by your own organisation or by a local authority. No ratio is printed here deliberately — establish yours and record it on the sheet.
