What this sheet is for

Keep a contemporaneous record of a shift, written as things happen rather than reconstructed at the end of it.

Who it is for

Security teams, caretakers, night staff, wardens, duty managers and anyone who hands a building over to somebody else.

How to use it

  1. Write entries as they happen, with the time.
  2. Plain language: what was seen, what was done.
  3. Record who you reported it to, and when.
  4. Initial every entry.
  5. Sign the handover at the end of the shift.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why does it matter when the entry was written?

Because a log written up at the end of a shift from memory carries far less weight than one written at the time, and anybody reading it later can usually tell which it was.
