Record not just that a contractor arrived but what they were told about your building before they started work.
Site managers, facilities teams, landlords, schools and anyone letting trades into premises they are responsible for.
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
That depends entirely on the site, the work and the law where you are, so the sheet prints no list. Write your own points in the space provided — the value is in covering the same things every time and having a record that you did.

Put a controlled job in writing: what is being done, what has to be true before it starts, who is covered and when it was handed back.
Record a short on-site briefing and, more usefully, what the people listening raised in response to it.
