  2. Write the starting eleven in shirt-number order with each player's registration number.
  3. Name the substitutes in the blank rows — as many as your competition allows.
  4. Mark goals, cards and substitutions as they happen.
  5. Enter the half-time and full-time score and have the manager and referee sign.

Questions

Why are there five substitute rows and no limit printed?

Because how many substitutes may be named and used differs by competition and changes between seasons. Five rows is the space on the page, not a rule; use as many as your competition allows and leave the rest blank.

Does this replace the match report?

No. The team sheet is what is exchanged before kick-off and marked during the game; the Match Report Form is what is written afterwards. The team sheet is the source for it.
