Record a fixture properly on the day, including the injuries and incidents that have to be reported afterwards.

How to use it
- Fill in the team before kick-off, then annotate as the game goes on.
- Record substitutions with times if your competition requires them.
- Write injuries and incidents down on the day, factually.
- Note who you sent the report to, and when.
- Keep a copy — competitions ask months later.
