What this sheet is for

Record a fixture properly on the day, including the injuries and incidents that have to be reported afterwards.

Who it is for

Team secretaries, coaches, managers and match officials in amateur sport.

How to use it

  1. Fill in the team before kick-off, then annotate as the game goes on.
  2. Record substitutions with times if your competition requires them.
  3. Write injuries and incidents down on the day, factually.
  4. Note who you sent the report to, and when.
  5. Keep a copy — competitions ask months later.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
